9/28/2023 EnchantaVaughn Approved 1. Behavior: Client struggled with coming to the session, as shown when he attempted to hide in his room and ignore QMHP presence. Client shared things he would like to do over what the QMHP instructed him to do. Client continued to give his counselor a hard time after being asked to correct his behaviors and show more respect. Client shared that he was not in a good mood. Client reframed from engaging in conversation and tried to cover his face, so he wouldn’t have to listen. 2. Behavior: Client often displays frequent outbursts when things don’t go his way. Client refused to process his feelings appropriately and continues to display a moody and hostile temper. Client was mostly quiet during the session and often zoned out, making it hard for him to stay focused. Client denied that he felt sad or depressed, despite the obvious signs (i.e. body language, tone, appearance, etc.). It is evident that part of client’s mood stems from lack of support and affection in his environment. 1. Intervention: QMHP retrained client on anger management techniques, as a way to help him regulate and regain confidence in reference to his mood-altering behaviors. QMHP intervened aggressive behavior and motivated client to act in a civilized way. QMHP utilized a reflective thinking exercise to help the client better understand and identify his triggers and stressors. QMHP directed client to verbally express his thoughts so that he can do a better job at implementing a healthier mental space. 2. Intervention: QMHP taught client relaxation, calming, and guided imageries, to help him reduce and regulate his overactive behaviors safely. QMHP provided and demonstrated positive emotions that can help client express himself when faced with hardships. QMHP implemented active listening skills. QMHP worked with client to reduce or eliminate harmful and hostile manner, within his daily functioning life. QMHP provided a summary of the session, to reassure client had clear knowledge of the purpose of session. 1. Response: Client exhibits a negative attitude when reflecting on his hardships and failures that are a part of his upbringing. Client failed to identify his triggers and stressors that are the cause of his mood swings. Client reframed from answering QMHP questions especially since it reminded him of his own mistakes. Client communicates a lot of sad feelings at this time. Client expressed he missed his Dad and dislikes having to choose which parent to live with. Client was filled with mixed emotions regarding this decision.
